Adult Day Centers

Required Documents

• Valid Center License
  A current license or certification required by applicable state law to provide adult day services (e.g., state license, Medicaid certification, or similar regulatory approval), along with a valid business license, if applicable. 
• W-9 most current version, signed in the current year https://www.irs.gov/forms-pubs/about-form-w-9
• Certificate of Insurance (COI): Your insurance certificate should name Bright Horizons as both Additional Insured and Certificate Holder and show at least $1M in general liability and physical/sexual abuse coverage. 
• Workers Compensation Certificate (if on a separate policy)
