SUPPLIER REGISTRATION FORMS
Through the Supplier Registration Process, prospective suppliers will be sent an invitation by a Bright Horizons employee to apply to become an approved supplier. This application will ask you to upload one or more of the forms listed below.
Federal Tax ID
IRS Form W-9 is used to gather your taxpayer identification number. By submitting this form, you are certifying that the tax id number you are providing is correct and accurate.
Foreign entities should attach the equivalent W-8 form. For information on how to complete a W-8 form, please consult the IRS.gov website.
Certificate of Insurance
Facilities and construction suppliers as well as those suppliers whose agents will come in direct contact with children are required to provide a Certificate of Insurance naming Bright Horizons and it's affiliate entities, as additional insured and certificate holder. The text should read:
Bright Horizons, Bright Horizons Children's Centers LLC and Bright Horizons Family Solutions are
included as Additional Insured on General Liability Insurance policy as it relates to the work performed by
the Named Insured

Enrichment Provider Agreement
Suppliers providing enrichment activities for children at our centers are required to sign our Enrichment Provider Agreement (EPA). This agreement must be also be signed by the Divisional Vice President for the center where the activities will take place. This agreement must be in place prior to any performance of work and is required for Procurement to setup the supplier for payment.
Supplier diversity is important to the mission of Bright Horizons and Nationally certified diverse suppliers are encouraged to self-identify during the registration process. Once registration is complete, Bright Horizons will reach out to confirm your diversity status.
At this time, Bright Horizons is able to recognize the following classifications:
- Certified minority-owned businesses, small and large
- Certified woman-owned businesses, small and large
- Certified small disadvantaged businesses
- Small veteran-owned and service disabled veteran-owned businesses
- Small, certified HUBZone businesses
- Gay and lesbian-owned businesses
